Skilled Nursing

Our skilled nursing care starts while the patient is in the hospital with an on-site hospital assessment. A thorough assessment will help us to prepare for a smooth transition to The Imperial. Private rooms based on availability. Upon discharge to the facility patients are personally evaluated by our interdisciplinary team and an attending physician. Available services include:

• Daily vital signs
• Medication monitoring according to established guidelines
• Necessary testing such as labs, EKG, and x-ray
• Nutrition Management
• Lifestyle modification
• Homebound site evaluation
 

Our goal is to optimize our patient’s condition so that a safe return home can be considered if appropriate. Long-Term care is available should it be needed. Our team will assist the patient and their family to decide on the next step.
 

Alzheimer’s and Memory Care

The Imperial Grove Pavilion provides person-centered care that accepts to unique needs, interests, and habits of those individuals diagnosed with Alzheimer’s disease and related dementias.
 

The Imperial Grove Pavilion is committed to offering a variety of daily activity programs that focus on the mind, body, and spirit of the dementia resident. Our professional recreational therapy team – as well as all other staff of The Imperial – understands the different functional levels of the resident through the dementia disease process and continually adapts daily activities to encourage a positive response from the resident.
 

Orthopedic Care

The Imperial’s physical therapists provide services that assist in restoring function, improving mobility, relieving pain and preventing or limiting permanent physical disabilities of patients suffering from injuries or disease. They restore, maintain, and promote overall fitness and health. The Imperial’s staff also teaches residents to use assistive and adaptive devices, when appropriate, such as crutches, prostheses, and wheelchairs. They also may show patients how to do exercises at home to expedite their recovery.
 

Specialized Wound Care

The Imperial Grove Pavilion is proud to offer wound care residents its SWAT program (Special Wound Aggressive Treatment). SWAT is a multi-system approach that uses advanced modalities and therapies which may include E-Stim, ultrasound, negative pressure therapies, MIST therapy, ultra violet therapy, and debridement.
 

SWAT candidates are individuals with complex wounds including pressure wounds, arterial wounds, neuropathic wounds, non-healing surgical wounds, venous and lymphedema related wounds, and burn wounds entering the rehabilitative stage.

 

Homeward Bound

Homeward Bound is The Imperial Grove Pavilion’s state of the art rehabilitation program. It is a comprehensive short-term physical rehabilitation program designed to return residents to their optimal functional level.
 

Our team of highly trained physical, occupational, and speech therapists work with our nurses and your doctor to create a personalized plan that addresses your needs, monitors your progress, and gives you the support you need for a speedy recovery.
 

The resident’s plan focuses on the individual, their goals, and outcomes through the use of nautilus equipment, strength training and pain management. Our therapy team works closely with our physiatrist to tailor a therapy program to each individual.
